Este o \u00eent\u00e2lnire de la care nu ne putem sustrage. Durerea e universal\u0103 \u0219i asta explic\u0103 faptul c\u0103 de mai bine de dou\u0103 mii de ani oamenii \u00ee\u0219i pun insistent \u00eentrebarea: Ce este, de fapt, durerea?\u201d<\/em> <em>(\u201dDurerea \u2013 Istoria convie\u021buirii cu un vechi du\u0219man\u201d<\/em>, carte semnat\u0103 Alexandru Babe\u0219, profesor de neuro\u0219tiin\u021be \u0219i fiziologie la Facultatea de Biologie a Universit\u0103\u021bii din Bucure\u0219ti. Pentru a \u00een\u021belege pe deplin conceptul de durere, recomand\u0103m cartea profesorului Alexandru Babe\u0219, o lectur\u0103 interesant\u0103 \u0219i apreciat\u0103)<\/p>\n\n\n\n<p>***<\/p>\n\n\n\n<p>De-a lungul vie\u021bii, cu to\u021bii experiment\u0103m unul sau mai multe <strong>episoade marcate de durere fizic\u0103<\/strong>. Dac\u0103 ar fi s\u0103 ne lu\u0103m dup\u0103 statisticile care au drept subiect central <strong>durerile de spate<\/strong>, descoperim c\u0103 8 din 10 persoane sufer\u0103, la un moment dat \u00een via\u021b\u0103, din pricina acestora. Tot conform statisticilor, circa 75% dintre persoanele adulte s-au pl\u00e2ns de dureri de cap \u00een ultimii ani, 30% dintre acestea fiind diagnosticate cu migrene. C\u0103 e moderat\u0103 sau sever\u0103, neuropatic\u0103 sau inflamatorie (vezi mai jos ce semnific\u0103 acestea), <strong>durerea este o experien\u021b\u0103 complex\u0103<\/strong>. \u00cen r\u00e2ndurile urm\u0103toare, vei descoperi <strong>cum este clasificat\u0103 durerea<\/strong>, care sunt <strong>cauzele<\/strong> care o declan\u0219eaz\u0103, cum poate fi <strong>diagnosticat\u0103<\/strong>, <strong>monitorizat\u0103<\/strong> \u0219i <strong>ameliorat\u0103<\/strong> eficient. \u00cens\u0103 nu ar fi corect s\u0103 vorbim despre durere f\u0103r\u0103 s\u0103-i amintim defini\u021bia.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\"><strong>Ce este durerea \u0219i care este mecanismul s\u0103u<\/strong><\/h2>\n\n\n\n<p>\u00cen anul 2020, <strong>Asocia\u021bia Interna\u021bional\u0103 pentru Studiul Durerii<\/strong> a revizuit <strong>defini\u021bia durerii<\/strong>, un standard care a fost respectat la nivel global \u00eenc\u0103 de prima oar\u0103 de c\u00e2nd a fost scris, \u00een anul 1979.<\/p>\n\n\n\n<p><em>\u201dDefini\u021bia revizuit\u0103 a fost un adev\u0103rat efort de colaborare, realizat de un grup operativ multina\u021bional, multidisciplinar, care a primit contribu\u021bii din partea numeroaselor p\u0103r\u021bi interesate, inclusiv a persoanelor care sufer\u0103 de durere \u0219i a \u00eengrijitorilor acestora\u201d<\/em>, a declarat Srinivasa N. Raja, MD, pre\u0219edinte al grupului operativ IASP\/International Association for the Study of Pain, profesor de anestezie &amp; medicin\u0103 de \u00eengrijire a pacien\u021bilor afla\u021bi \u00een stare critic\u0103, profesor de neurologie la \u0218coala de Medicin\u0103 a Universit\u0103\u021bii John Hopkins.<\/p>\n\n\n\n<p>Conform noii defini\u021bii, durerea este descris\u0103 astfel:<\/p>\n\n\n\n<p><em>\u201eO experien\u021b\u0103 senzorial\u0103 \u0219i emo\u021bional\u0103 nepl\u0103cut\u0103 asociat\u0103 cu, sau asem\u0103n\u0103toare cu cea asociat\u0103 leziunilor tisulare reale sau poten\u021biale\u201d<\/em>, \u0219i este extins\u0103 prin ad\u0103ugarea a \u0219ase puncte cheie, dup\u0103 cum urmeaz\u0103:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>durerea este \u00eentotdeauna o <strong>experien\u021b\u0103 personal\u0103<\/strong> care este influen\u021bat\u0103 \u00een diferite grade de <strong>factori biologici<\/strong>, <strong>psihologici<\/strong> \u0219i <strong>sociali<\/strong>;<\/li>\n\n\n\n<li>durerea \u0219i <strong>nocicep\u021bia<\/strong> sunt fenomene diferite. Durerea nu poate fi dedus\u0103 numai din activitatea neuronilor senzoriali;<\/li>\n\n\n\n<li>prin experien\u021bele lor de via\u021b\u0103, oamenii se familiarizeaz\u0103 cu <strong>conceptul de durere<\/strong>;<\/li>\n\n\n\n<li>raportul unei persoane la o experien\u021b\u0103 \u00een care a fost implicat\u0103 durerea trebuie respectat;<\/li>\n\n\n\n<li>de\u0219i durerea are, de obicei, un <strong>rol adaptativ<\/strong>, poate avea efecte adverse asupra func\u021biei \u0219i bun\u0103st\u0103rii sociale \u0219i psihologice;<\/li>\n\n\n\n<li>descrierea verbal\u0103 este doar unul dintre multiplele comportamente de exprimare a durerii; incapacitatea de a comunica nu anuleaz\u0103 posibilitatea ca un om s\u0103 experimenteze durere.<\/li>\n<\/ul>\n\n\n\n<p>Defini\u021bia revizuit\u0103 a fost publicat\u0103 \u00een jurnalul oficial al asocia\u021biei \u2013 <strong><em>PAIN<\/em><\/strong> &#8211; \u00eempreun\u0103 cu comentariul pre\u0219edintelui Lars Arendt-Nielsen \u0219i al fostului pre\u0219edinte, Judith Turner.<\/p>\n\n\n\n<p><em>\u201eIASP\/ International Association for the Study of Pain \u0219i grupul de lucru care a scris defini\u021bia revizuit\u0103 au f\u0103cut acest lucru pentru a transmite mai bine nuan\u021bele \u0219i complexitatea durerii \u0219i au sperat c\u0103 acest lucru va duce la o evaluare \u0219i o gestionare \u00eembun\u0103t\u0103\u021bite a celor care sufer\u0103 din pricina durerii\u201d<\/em>, a spus dr. Durerea este un monstru cu multe chipuri.\u201d<\/em> (\u201d<em>Durerea \u2013 Istoria convie\u021buirii cu un vechi du\u0219man\u201d<\/em>, Alexandru Babe\u0219)<\/p>\n\n\n\n<p>Nepl\u0103cuta \u0219i deranjanta durere, deseori cauzat\u0103 de stimuli cu intensitate ridicat\u0103 sau care provoac\u0103 leziuni, func\u021bioneaz\u0103 aidoma unui mecanism de avertizare sau de alertare a organismului cu privire la poten\u021bialele pericole la care este expus. Se vorbe\u0219te despre patru mecanisme care influen\u021beaz\u0103 \u0219i ac\u021bioneaz\u0103 durerea: fibrele nervoase, receptorii durerii, m\u0103duva spin\u0103rii \u0219i creierul. Odat\u0103 cu primirea unui stimul fizic, sunt activa\u021bi receptorii nociceptivi ai durerii, \u00een timp ce fibrele nervoase declan\u0219eaz\u0103 semnalul de alarm\u0103 pentru receptorii durerii. Semnalele ajung la creier, prin intermediul nervilor de la nivelul m\u0103duvei spin\u0103rii, iar ulterior \u00een talamus. Originea durerii este identificat\u0103 cu ajutorul cortexului. Trebuie s\u0103 se \u021bin\u0103 cont de faptul c\u0103 resim\u021birea durerii este subiectiv\u0103, difer\u0103 de la persoan\u0103 la persoan\u0103, poate fi afectat\u0103 de emo\u021bii, context personal\/social\/etc.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\"><strong>Care este rolul durerii<\/strong><\/h2>\n\n\n\n<p><em>\u201dDurerea&nbsp;are dou\u0103 scopuri principale: primul este s\u0103 ne \u00eenve\u021be cum s\u0103&nbsp;evit\u0103m stimulii sau situa\u021biile periculoase, iar al doilea este s\u0103 \u00eencurajeze recuperarea, dup\u0103 ce am e\u0219uat \u00een \u00eenv\u0103\u021barea primei lec\u021bii.\u201d<\/em> (\u201d<em>Niciodat\u0103 nu e de-ajuns. Neuro\u0219tiin\u021ba \u0219i experien\u021ba dependen\u021bei<\/em>\u201d, Judith Grisel)<\/p>\n\n\n\n<p>Este foarte adev\u0103rat c\u0103 durerea joac\u0103 un rol important \u00een vie\u021bile tuturor, iar primul este acela de a ne proteja prin simplul fapt c\u0103 ne permite s\u0103 asociem senza\u021bia de durere cu o ac\u021biune d\u0103un\u0103toare, cu o afec\u021biune care, netratat\u0103 corespunz\u0103tor \u0219i la timp, se poate agrava.<\/p>\n\n\n\n<p><em>\u201dSpre deosebire de celelalte sim\u021buri,&nbsp;durerea&nbsp;este critic\u0103 pentru supravie\u021buirea&nbsp;noastr\u0103, iar senza\u021bia seam\u0103n\u0103 mai cur\u00e2nd cu o lentil\u0103 personal\u0103, care poate&nbsp;deschide \u0219i \u00eenchide por\u021bile experien\u021bei senzoriale. De exemplu, orbii, surzii sau&nbsp;anosmicii (ultimii sunt cei c\u0103rora le lipse\u0219te mirosul) au \u0219anse mari s\u0103 tr\u0103iasc\u0103&nbsp;o via\u021b\u0103 normal\u0103, dar acesta nu este \u0219i cazul celor n\u0103scu\u021bi cu o insensibilitate&nbsp;congenital\u0103 la durere, care aproape invariabil mor tineri, de la complica\u021biile&nbsp;ap\u0103rute \u00een urma unei r\u0103niri.\u201d <\/em>(\u201d<em>Niciodat\u0103 nu e de-ajuns. Neuro\u0219tiin\u021ba \u0219i experien\u021ba dependen\u021bei<\/em>\u201d, Judith Grisel)<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\"><strong>Cele mai \u00eent\u00e2lnite tipuri de durere<\/strong><\/h2>\n\n\n\n<p>C\u00e2nd vine vorba de durere, se vorbe\u0219te despre cinci tipuri care sunt \u0219i cele mai \u00eent\u00e2lnite: <strong>durerea acut\u0103<\/strong>, <strong>durerea cronic\u0103<\/strong>, <strong>durerea nociceptiv\u0103<\/strong>, <strong>durerea neuropatic\u0103<\/strong> \u0219i <strong>durerea radicular\u0103<\/strong>. Iat\u0103, foarte pe scurt, care sunt principalele tr\u0103s\u0103turi ale acestora.<\/p>\n\n\n\n<p><strong>Durerea acut\u0103<\/strong> este acea durerea sever\u0103 sau care apare subit, resim\u021bit\u0103 c\u00e2nd \u00een organism exist\u0103 o afec\u021biune sau \u00een urma unui traumatism\/interven\u021bie chirurgical\u0103\/lucr\u0103ri dentare\/travaliu-na\u0219tere\/arsuri \u0219i t\u0103ieturi, etc. \u00centr-o anumit\u0103 perioad\u0103 de timp, care variaz\u0103 \u00een func\u021bie de cauza apari\u021biei durerii, durerea acut\u0103 va disp\u0103rea. Este influen\u021bat\u0103 de factori psihologici \u0219i de mediu \u0219i poate afecta deopotriv\u0103 via\u021ba social\u0103 \u0219i personalitatea persoanei \u00een suferin\u021b\u0103. &nbsp;<\/p>\n\n\n\n<p><em>\u201dC\u00e2nd durerea devine cronic\u0103, ea \u00eenceteaz\u0103 s\u0103 mai fie un semn de avertizare \u0219i devine ea \u00eens\u0103\u0219i boala. Aceasta poate fi acut\u0103 sau cronic\u0103.<\/p>\n\n\n\n<p>La polul opus se situeaz\u0103 <strong>durerea neuropatic\u0103<\/strong> asociat\u0103 cu <strong>leziunile nervilor<\/strong> sau ale altor p\u0103r\u021bi ale sistemului nervos, fiind descris\u0103 ca o durere arz\u0103toare sau senza\u021bia de ace \u00eenfipte \u00een corp.<\/p>\n\n\n\n<p><strong>Durerea radicular\u0103<\/strong> apare c\u00e2nd <strong>nervul spinal este inflamat sau comprimat<\/strong> \u0219i poate fi \u00eenso\u021bit\u0103 de furnic\u0103turi, sl\u0103biciune muscular\u0103, amor\u021beal\u0103. Una dintre cele mai \u00eent\u00e2lnite forme de durere radicular\u0103 este cea care apare ca urmare a afect\u0103rii nervului sciatic (radiculopatie) fiind durerea care iradiaz\u0103 din spate \u0219i \u00een picior, o durere constant\u0103 care ia amploare fie la mersul pe jos, fie \u00een urma statului pe scaun timp \u00eendelungat.<\/p>\n\n\n\n<p><em>\u201dDurerea cronic\u0103 are darul de a distruge nu doar via\u021ba trupului, ci \u0219i pe cea a sufletului, fiind \u00eenso\u021bit\u0103 aproape \u00eentotdeauna de anxietate, depresie \u0219i o reducere a calit\u0103\u021bii vie\u021bii care ni se pare intolerabil\u0103. Omul medieval c\u0103uta \u00een mod deliberat durerea pentru a\u2011\u0219i salva sufletul nemuritor de tenta\u021biile c\u0103rnii; omul modern pretinde ca medicina s\u0103\u2011i curme instantaneu durerea. \u0218tiin\u021ba a pus telescopul James Webb pe o orbit\u0103 aflat\u0103 la 1,5 milioane de kilometri de P\u0103m\u00e2nt, a descoperit bosonul Higgs \u0219i a ajuns \u00een pragul edit\u0103rii genomului uman. Alexandru Babe\u0219)<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\"><strong>Care sunt cauzele care duc la instalarea durerii<\/strong><\/h2>\n\n\n\n<p>Pornind de la clasificarea durerii, aceasta se instaleaz\u0103 atunci c\u00e2nd anumite termina\u021bii nervoase sunt stimulate, c\u00e2nd apare probleme la nivelul nervilor \u0219i c\u00e2nd \u00een peisaj exist\u0103 o afec\u021biune medical\u0103 care trebuie \u021binut\u0103 sub observa\u021bie \u0219i remediat\u0103. Printre cele mai \u00eent\u00e2lnite afec\u021biuni responsabile pentru durere se num\u0103r\u0103: endometrioza, fibromialgia, bolile inflamatorii intestinale, diverse infec\u021bii, disfunc\u021bii articulare, cistita intersti\u021bial\u0103, etc.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\"><strong>Cum se m\u0103soar\u0103 durerea<\/strong><\/h2>\n\n\n\n<p>\u0218tiai c\u0103 durerea are o unitate de m\u0103sur\u0103? Aceasta se nume\u0219te \u201ddol\u201d, iar aparatul cu care se m\u0103soar\u0103 (at\u00e2t pragul durerii, c\u00e2t \u0219i toleran\u021ba) poart\u0103 denumirea de \u201ddolorimetru\u201d.
